Traditionally, adjacency in Birthright is accomplished in one of two ways:
Land an army using ships (ridiculously difficult) or,
Provinces separated by a single ocean zone are considered adjacent.
However, neither really works here because we don't have concrete units and ships, and because most of the island provinces are dwarven and were traditionally connected by tunnels.
While southern Albion won't come into play (unless the game ends or is ending), other islands are a convenient means of expanding the game if we get enough players (Hibernia, Faroes, Iceland).
So do we handle adjacency by culture? And for longer distances, how is that handled?
One possibility is "Trade Routes" - basically, all property-style holdings may connect provinces by sea. Properties did need additional benefits. Thematically:
Manors attract labor to/from distant lands, Sanctums attract pilgrims, Trade is self explanatory, Nodes are the magical version of trade
But then how do you connect province A and B in such a manner in the first place? Especially in an involuntary situation.

I think you should do like in Risk, where you predetermine adjacency between the various provinces, and disregard everything called Ocean Adjacency. I know that some provinces will have "Ocean Adjacency" in this manner, but rule wise it'll simply work as normal adjacency.

That could work and would potentially work with the "trade routes" idea as well, i.e. if you can establish a trade route somehow (most likely diplomacy or adventure), then you can establish "ocean adjacency". Conversely, you can disestablish "ocean adjacency" in a similar manner, or it can be disestablished by events.
